Who's eligible for individual insurance plans
Personal Health Plans Dental plans are available to all ages and maybe purchased in conjunction with a medical plan or as a stand-alone product.
You and any dependents may apply for Consumers Life insurance as long as you are residents of your state at least six months of the year.
Family coverage includes you, your spouse and any unmarried children under the age of 19; 23 for full-time students. Dependent-only coverage is available.

Let the dentist know that you are a Personal Health Plans from Consumers Life member. Then present your identification card at your first visit.
A claim will be submitted to Consumers Life for the services you received
