With our personal vision insurance you have annual benefits towards an eyeglass examination and pair of eyeglasses or contact lenses, available through thousands of participating network providers. Reduced benefits are available when you obtain services outside the vision care provider network.

Who's eligible for individual vision insurance plans |

- Personal Health Plans Vision plans are available to all ages and maybe purchased in conjunction with a medical plan or as a stand alone product.
- You and any dependents may apply for Personal Health Plans from Consumers Life insurance as long as you are residents of your state at least six months of the year.
- Family coverage includes you, your spouse and any unmarried children under the age of 23. Dependent-only coverage is available.
