All Personal Health Plan medical plans (Standard, Value, HSA-Compatible HSA-Compatible Wellness, and Short-term) contain prescription drug coverage.
When enrolling in a Value medical plan, you can upgrade your prescription drug coverage by choosing this rider.

• With this Personal Health Plan Optional Prescription Drug Rider, your out-of-pocket costs will be limited to a lower copay for each generic, formulary and non-formulary prescription filled.
• Pay the single copay for retail or for home delivery, pay two times the copay for a 90-day supply.
• The annual deductible and co-insurance are waived.
