Who's eligible for personal dental insurance plans
Personal Health Plans dental coverage is available to all ages and maybe purchased in conjunction with a medical plan or as a stand-alone product.
You and any dependents may apply for Consumers Life insurance as long as you are residents of your state at least six months of the year.
Family coverage includes you, your spouse and any unmarried children under the age of 23. Dependent-only coverage is available.

Call a DenteMax dentist to schedule an appointment.
Let the dentist know that you are a Personal Health Plans from Consumers Life member. Then present your identification card at your first visit.
A claim will be submitted to Consumers Life for the services you received
